The Palliative Care Community Team

Multidisciplinary team providing in-home care and support to residence of Haliburton County living with a life-limiting disease

• support to patients and their families can begin as soon possible and are designed to support the clients end of life journey

The PCCT Nurse Navigator & Circle of Care

Care partner and link to the health care team;

• shared care plans * assessment * referral
• access to palliative care, grief, bereavement and support services

Care team can involve clients primary care practitioner, specialists, care coordinator, and hospice programming

Patient supports:

• physician access - 24 hours a day/7 days a week (through nursing agency)
• volunteer visiting
• coordinated support to health services
• access to pain and symptom management
• client centered inter-professional case coordination

The Palliative Care Community Team is a partnership of Haliburton Highlands Health Services, Haliburton Highlands Family Health Team, Central East Local Health Integration Network and Peterborough Hospice Palliative Pain and Symptom Management Consultation Services.

• Note: Referrals are reviewed Mon-Thu 9 am-1 pm. Sending in the referral form does not automatically mean the patient has been accepted for service
