Description (Service) COVID-19 Response During the COVID 19 Pandemic, the South Georgian Bay Community Health Centre (SGBCHC) continues to offer virtual and in-person appointments for specific services. When your appointment is made, they will determine whether an in-person visit or telephone/video appointment is more appropriate.

Regular Service: 
A non-profit, community governed, charitable organization that focuses on the health and well being of individuals in the community * Health professionals include: a physician, nurse practitioners, registered nurses, a registered dietitian, diabetes educators, a mental health and addiction counsellor, a physiotherapist, a health promoter and social workers 
Provides access to health care services, free community programs and community development * Also offers referrals to other local health services and personal development groups 
Priority will be given to individuals and families that are currently without a doctor, and who fall into one of the following priority populations: 
* Individuals or families experiencing poverty, housing and/or food security challenges 
* Youth, between the ages of 12 and 24, with an emphasis on at-risk youth 
* Isolated seniors 
* Individuals with disabilities that present barriers to health care service access 
* Individuals or families experiencing mental health and/or addiction issues 
Note: This is NOT a walk-in clinic 
Services with a Nurse Practitioner are also offered to registered clients in Elmvale on Wednesdays by appointment at the Springwater Health Services Building (Kenzie Knox Building - 35 Queen Street West, Elmvale) from 9 am-3 pm, see separate entry for further details

Application For new patients in need of primary health care, call reception to get on the waitlist * Partner organizations can refer clients with more urgent needs by calling the main line
