Description (Service) Provides inter-professional assessment, counselling and education introducing tools and skills for adults to self-manage their chronic disease
  • clinics include Asthma Clinic, Adult Diabetes Education Clinic, Chiropody Clinic (limited to diabetics with foot ulcers), Geriatric Clinic, COPD Clinic, Internal Medicine Clinic and Stroke Prevention Clinic
  • Clinics may provide diagnostic testing, risk factor assessment (including prevention of falls) and treatment

  • Age Requirements: 18 years and up
  • Adults with chronic disease or at risk of developing chronic disease
Application
  • For Diabetes Clinic only: self-referral or medical referral
  • for all other clinics a medical referral is required
